Looks like not only was the nude photoshopped into the frame (see the shadows, different colour…) but the heads may have been photoshopped on as well (anyone recognise them, this is done frequently with celebrities?).
The dress looks nice, btw., her pose looks like something you’d see on a catwalk, maybe that’s where her photo comes from and it’s photoshopped in as well.

I found the source image. So the clothed woman is really there, except her face has been changed. The nude woman has been inserted to replace another clothed one. I’m inclined to think she’s entirely fake, a 3D render. Most likely Poser, as Nemo suggested. Although there could be some real photography mixed in too. And the background was also changed for some reason. How odd.
